Safety & Liability Waiver

This is a legally binding acknowledgment of risk, waiver, release from liability, and indemnity agreement. By signing this agreement,
the participant (and if applicable, the participant’s parent or legal guardian) agrees to waive and release Bingham Party Rentals LLC,
its owners, officers, employees, agents, contractors, and affiliates from all liability related to the Mechanical Bull attraction.

ASSUMPTION OF RISK:
I understand that participation in the Mechanical Bull attraction carries with it inherent risks, including but not limited to the risk
of personal injury, falls, equipment malfunctions, collisions, or the actions of other participants. I voluntarily assume all risks
associated with my (or my child’s) participation.

RELEASE FROM LIABILITY:
I, on behalf of myself and/or my minor child, hereby waive, release, and discharge Bingham Party Rentals LLC from any and all claims
for damages, injuries, losses, or liabilities that may arise out of or in connection with participation in the Mechanical Bull attraction.

INDEMNIFICATION:
I agree to indemnify and hold harmless Bingham Party Rentals LLC against any and all claims, actions, damages, costs, or expenses,
including attorney’s fees, arising from or related to my (or my child’s) participation in this activity.

ACKNOWLEDGMENT AND AGREEMENT:
I certify that I am at least 18 years old and legally competent to sign this agreement or am the parent/legal guardian of the minor
participant named below. I acknowledge that I have read, understood, and agree to all terms stated in this waiver.
